PAYMENT

Sessions are one hour long. Arrangements can be made for longer sessions when requested. This may be especially helpful for individuals and couples coming from a great distance or who have scheduling difficulties. Weekend and evening hours available. 

Some insurance is accepted. However, because of our commitment to making care accessible and due to the complexity and expense of receiving reimbursement through some insurers, we may not be able to accept your insurance. Therefore, we have constructed an affordable fee-for-service schedule due at the time of service.  A receipt will be provided with a medical diagnostic code should you choose to submit this to your insurance company or an HSA account through your employer for reimbursement directly to you. 

Sliding fee schedule for individuals is based on an hourly rate of $80.00-$120.00. 

Sliding fee schedule for couples generally reflects a 90 minute session with a rate of $150.00-$225.00

Thirty minute medication visits: $50.00-$75.00

POLICIES

Controlled Substances and Medications: Early refill requests will generally not be granted. Overuse or failure to keep medication safe may require discontinuation of a prescription. Any active drug or substance abuse must be addressed at the time of your initial visit for your safety and for an effective medication plan to be formulated. All medications which are prescribed must be used as agreed upon in our appointments for your safety. It is critical that you bring a current medication list with dosages for review at the initial visit and that you keep us updated with any changes.

Collateral Communication with other members of your healthcare team (current and former) may be necessary and in the best interest of comprehensive care. This will be discussed at the initial visit.

Cancellations: 

Full self-pay fee will be applied for appointments cancelled within less than 48 hours. This will apply for missed appointments as well. We realize that weather and health emergencies occur and will be taken into consideration.
